- Marker writing on tag has a #5 and a G but player unknown
- #5 patch on the right arm and Sky Chiefs logo in on the left arm
- Syracuse Chiefs changed their name to the Sky Chiefs in 1997 until 2006 when they changed it back to the Syracuse Chiefs (unitl 2019, when they changed their name again to the Syracuse Mets)
- They were the AAA affiliate of the Toronto Blue Jays for the entirety of thier Sky Chiefs era

- Excellent condition; #5 G written in ink on the tag otherwise little to no signs of visible handling wear
