- St. Catharines Black Hawks: the organization existed from 1962 to 1976, and were a junior ice hockey team part of the Ontario Hockey Association. The NHL's Chicago Blackhawks sponsored the St. Catharines Teepees and renamed them, a sponsorship lasting from 1962 - 1968
- London Nationals: the Nationals, based in London Ontario, were founded during the 1950s and remain active today. They have had seven different organization names, and have won 14 championships across a variety of different tournaments
- Very good condition: some light creasing throughout, staining on the lineup pages and discolouration along the edges. General edgewear throughout but no rips or tears
