- Joined the NHL in 1979 as one of the four teams it absorbed, renamed to the Hartford Whalers
- Relocated to North Carolina in 1997, becoming the Carolina Hurricanes
- Plaques were issued by WHA front office to its member teams, each displaying their logos
- 5.25 x 5.25"
- Excellent condition; adhesive residue in upper right corner, light scuffing throughout
